Sponsor Letter

PLAYER FORMS: Each player will need the following:

1. A registration form:

Boys & Girls Basketball

Baseball
Girl's Softball

2. A sports physical form, signed by a doctor, OR...

3. A waiver of physical form, if you opt not to do a physical,
signed by a parent.

4. A volunteer form

5. Final Fees. (See Registration form) with check payable to
"Victors Homeschool Sports."

You can download the above forms, fill them out, and bring them to the first practice.
